Several surgical techniques are available to reconstruct the nipple and the surrounding areola. One method involves taking a small skin flap from the existing breast and forming it into a nipple. What we commonly likely to refer to as "skin origami." Since the reconstructed breast does not contain active nerve endings, the procedure causes little to no discomfort.

A flap of skin may taken from another area of the body, such as the inner thigh or groin crease. This can be used to reconstruct the areola. It is normal for the site where the skin was harvested to be a little sore for a couple of weeks following the procedure; however, there is typically little discomfort around the grafted site. An alternative to grafting is to tattoo the skin around the reconstructed nipple to mimic the appearance of an areola. This is our preferred technique which minimizes donor site scars and recovery.

What Occurs After this Procedure

A nipple reconstruction is an outpatient procedure or can be performed in the office. It is normally completed under local anesthesia in approximately one hour. Most of our patients return to work within a few days of this surgery as there is little to no downtime when performed alone. However, occasionally we will combine it with other procedures which may add time to recovery.

Once the area is sufficiently healed, the nipple and surrounding areola can be tattooed for a more natural appearance. The tattooing typically requires two to three sessions lasting about 90 minutes with each session. The procedure is generally seen to be pain free with topical medications. We aim to make you as comfortable as possible during this procedure.

Most women feel less self-confident when their reconstructed breast does not match their natural breast. A nipple reconstruction makes it possible to create a reconstructed breast that closely resembles a natural breast. The downside is that it does involve a second surgical procedure along with recovery time. There will also be some scarring at the site where the skin used to create the nipple is harvested.

Can nipple reconstruction be performed during breast reconstruction?

Generally we like to wait until the final step to perform nipple reconstruction. In rare cases, such as oncoplastic reconstruction this may be performed earlier on. Dr. Priti Patel commonly suggests waiting to perform nipple reconstruction surgery so that your breasts have a chance to heal. This will allow for your reconstructed nipples to more closely match your breasts after an adequate amount of time has passed to allow your body to settle the reconstructed breasts into their final position. When performing one side, this also allows us to closely match the opposite side. Generally we will wait approximately 3 months for this to occur.

Is nipple reconstruction surgery permanent?

Nipple reconstruction surgery is considered a permanent surgery. In some cases, nipples can flatten over time. We can consider reshaping the nipple mound if this occurs or even perform 3D nipple tattoo procedures.
